Confessions is voornamelijk een R&B album en laat Usher zien als een crooner door middel van een mix van ballads en up-tempos, met muzikale genres als dance-pop, hip hop en crunk. De thema's van het album veroorzaakten controverse over Usher's persoonlijke relaties; de hoofdproducer van het album, Jermaine Dupri, beweerde echter dat het album Dupri's eigen persoonlijke verhaal weerspiegelt
